FORMS
If you have made in‑kind contributions — such as expenses for advertising or advocacy on behalf of the Michigan Citizen Only Voting -Michigan ballot initiative — please download the attached PDF form, fill it out, and return it with copies of your receipts. You may scan and email the completed form and documentation to info@michcov.com, or mail it to the address listed on the form.
These contributions will be reported in accordance with Michigan campaign finance laws.
